About Adult Services Block 1

We support people over the age of 18 with a physical or sensory impairment, mental health issues, or a learning disability. Whoever they are, we make sure the person is at the heart of everything we do.

Composed of highly professional social care workers, our adult care operational teams work alongside adults with care and support needs, helping them live their best lives. We give our staff the opportunity to learn, develop, and grow. Our induction process for new starters is excellent, and you will receive regular supervision and training, access to memberships and subscriptions such as Research in Practice and Social Care Information and Learning Services (SCILS), team meetings, peer groups, journal clubs – the list goes on!

Community Support

This service, launched in 2021, covers the whole of the county and focuses on supporting people in a strengths-based way, helping to develop and strengthen their networks and engage with community-based support where appropriate.

The Mental Health Social Work service provides specialist services for adults who are experiencing acute or chronic mental health issues. We have three locality teams based across Wiltshire and also have a county-wide Approved Mental Health Professional Service. Our main focus of work involves care management for mental health-related needs, forensic Social Work with a Mentally Disordered Offender cohort, statutory Mental Health Act work, and supporting the Adult Multi-Agency Safeguarding Hub undertake safeguarding investigations. The teams collaborate closely with allied health professionals as well as other agencies involved in the field. We aim to support people to stay as independent as possible using a person-centered, strengths-based approach. We offer excellent training, learning, and development opportunities for our staff including an AMHP qualification pathway.

The Adult Multi-Agency Safeguarding Hub (MASH) works in partnership with a number of agencies and individuals to ensure that ‘Adults at Risk’ are supported appropriately and that their outcomes are central to any Safeguarding enquiries. We work alongside people to identify what is important to them, focusing on ‘Making Safeguarding Personal’ (MSP), and embedding the Care Act ‘Safeguarding Principles’ within all enquiries.

The Adult MASH aims to support the adult’s right to live in safety, free from abuse, and neglect. We work together with organisations to prevent and stop both the risks and experience of abuse or neglect, while at the same time making sure that the adult’s wellbeing is promoted including, where appropriate, having regard to their views, wishes, feelings, and beliefs in deciding on any action.

The Learning Disabilities and Autism Service is a service that aims to support people with learning disabilities and/or autism to lead their best lives. The team ensures that people who require social care support are at the centre of decision making about their own lives. This principle underpins everything that the service does.

The service has three priorities:

  • Supporting people to be independent
  • Enabling people to achieve their goals
  • Advocating with people who have learning disabilities and/or autism

Services to support people at home

The hospital discharge teams are based in three acute hospitals and provide advice, assessment and discharge planning for people. All of these teams work closely with health colleagues to promote an effective multi-disciplinary approach. We're currently looking at innovative new methods such as 'discharge to assess' which is following people from discharge to their own home or care home. We're also exploring innovative front door of hospital approaches to ensure the correct discharge pathway is instigated as soon as possible.

The aim of the Reablement Service is to work with people in the community in a person-centred and positive way to live the best life they can.

Reablement focuses on what people can do and builds on these skills rather than focusing on what the individual cannot do. Reablement also works with community partners and organisations to re-connect the individual to activities and groups that have been or are important to that person. This improves well-being and reduces loneliness.

Unlike traditional domiciliary care which assists an individual to carry out their day to day activities such as washing, dressing, using the toilet and preparing meals Reablement takes a person-centred approach. The individual sets their own goals to be achieved over a short time period and the Reablement worker helps them to achieve these goals through encouragement and support.

We're a new service who provide short term care to those that have been discharged from hospital or those in crisis requiring a rapid response to prevent hospital admission.

Wiltshire Support at Home promotes, wherever possible, the independence of the people receiving a service, ensuring they have a positive experience which maintains their mental and physical well-being.

Living Well

The ongoing Support teams help those most in need by listening and responding to the specific needs of individuals. We work alongside people to identify what is important to them and support our customers to make a plan to improve things that aren’t going so well so that they can live the life they want to.

We work with health partners to help people before they reach crisis point. We focus on identifying existing individual, family and community support, ensuring that people can live independently for as long as possible.

Rather than assuming responsibility for providing care, we look for local, sustainable and less intrusive solutions wherever they are available. We help people access the wide range of opportunities for intervention and prevention that are provided.

The hearing and vision team is dedicated to supporting people who have sensory loss or impairment. Our specialists in hearing and vision work county-wide and are focused on promoting independence, equality and inclusion by providing a service which enables a person to have choice and control over their life. Specialist knowledge and skills are used to work with the person, their carer, family, community and services to achieve a positive outcome for them and improve their quality of life.
