About Block 1

M&D Care has been an established care provider since 2008. We are a family-oriented business, which is run by a passionate team of professionals who deliver specialist, bespoke Residential, Supported Living and Domiciliary Care services throughout South West Wales for people with Autism, Learning Disability, Physical Disability and Mental Health needs. We aim to offer a high standard of service, to make a fundamental and lasting improvement in the quality of life for individuals requiring support. We believe that diagnosis is no barrier, and the most important part of our service is empowering people to face their individual challenges and providing support to enable them to become more independent and live fulfilling lives.

Here at M&D Care we are passionate about providing support for individuals with complex needs, many of whom have experienced multiple community placement breakdowns and/or prolonged hospital admissions due to complexity and risk relating to their behaviours. Our aim has always been to promote maximum independence and community integration within safe, nurturing environments. We believe in the potential of everyone and the enablement of working towards goals, and aspirations in life however big or small these may be.

About Block 2

Company History

Our Company Developments Director, Michelle Martin, is an experienced psychiatric nurse and is one of the company founders being the ‘M’ in M&D Care. Michelle states that the main motivation for starting M&D Care was realising that people need to live in homes, not hospitals. She acknowledges that whilst she is extremely proud of M&D Care’s growth, she is also very proud of its history - “At M&D Care we understand where we have come from, which is important as we remain close to our core values and founding principles”.

Our first residential service opened in 2008 to be followed over the years by the further development of residential homes across the Carmarthenshire and West Glamorgan areas. To complement our residential services, we opened both our Supported Living and Domiciliary Care services in 2015. This enables us to provide a progression pathway, to help people plan for their futures or to provide specialist intensive support including personal care in people’s own home or tenancies. It is only possible to offer this growth and increase in service flexibility with a continued focus on the quality of the support we provide. The company continues its focus on sustainable quality development and growth.

About Block 3

Specialist Residential Support Services

The Specialist Residential Support Services are designed with an infrastructure of technology, and adaptability to support an individual to live a least restrictive way of life. In 2018, M&D Care commenced work to develop specialist support services for individuals with learning disabilities and very complex behavioural support needs. Advice and support were provided by lead professionals in learning disabilities in South Wales. The aim was to develop an in-house team of trained, experienced and skilled specialist practitioners and clinicians to provide bespoke specialist advice and support to M&D Care specialist residential services, including embedding PBS and least restrictive practice across all company services.

In early 2020 M&D Care opened Ynystawe Lodge, the first specialist flat-led residential model support service within a least restrictive environment. Ynystawe Lodge was specifically designed to enable individuals to have their own robust individual living space, whilst also offering them opportunities to develop tolerance and coping skills at their own pace, through the careful planned use of communal living areas.

Following on from the success of this flagship service M&D Care identified the need to develop a similar specialist bespoke service in Pen-y-groes, Llanelli which opened its doors in June 2021. Ty Hiraeth was developed with the same in-house support structure and flat-led model of support and is built around individual needs and is designed to be adapted to meet the bespoke requirements of the person requiring support.

The inception of Tegfan comes from the feedback received from individuals, families, and care teams and along with evidence of improved quality of life outcomes for the individual’s supported. Like Ynystawe Lodge and Ty Hiraeth, PBS practitioners will work full-time within Tegfan to ensure ongoing practice leadership and to embed PBS and Active Support into their everyday lives. Additional support will also be provided in-house through M&D Care’s specialist support services, including qualified learning disability nurses and PBM trainers.
