The Council are looking to appoint an enthusiastic, committed and innovative social worker to join their friendly and established single point of access team in order to support citizens across Southend-on-Sea. This position will be to assess and safeguard citizens in the community at the first point of contact with social care. The team is integral in safeguarding citizens in Southend, preventing admission to hospital and ensuring that citizens are linked to their local communities. The team are required to ensure that individual strengths, assets and wishes are maximised and potential for independence is maximised.You need to be committed to actively supporting independence, promoting wellbeing, and ensuring vulnerable Southend residents are safeguarded against abuse.

What you'll need to succeed
To be successful in this position it is essential that you have a qualification in Social Work (Diploma/Bachelor/Master’s degree) and hold a current registration with Social Work for England. It is essential that you possess a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005. The post holder will need to have a sound knowledge of the SET Procedures for Safeguarding Vulnerable Adults and assessment and investigation skills. You will need to demonstrate that you have a proven ability in ensuring that adults and older people are appropriately safeguarded. It is desirable that you have some hospital and/or community experience to understand the interface and delivery of care between Health and Social Care, to include a working knowledge of Continuing Healthcare.
