It’s exciting times for us here at Southend Care as we take on new teams of people that we can train, support and develop to flourish in the roles of supporting and caring for the community. We can offer flexible shift patterns ranging from a bank contract up to 36 hours per week which will suit individuals needing flexibility. Our Community Carers work 6 hour shifts 7am-2pm and 4pm-11pm, including bank holidays and weekends. However, we able to offer a range of flexible working rotas. Our rotas are set out 14 days in advance and we try to allow for every other weekend off.

Since the start, we have developed a strong track record as a local care provider working in close partnership with Southend Borough Council, the NHS and the local community. You will be re-reimbursed for your travel and enjoy meeting residents in their homes as well as in community facilities. Community Carers

The Role:

You will be providing person centred care and support to help individuals regain skills to live independently and safely in their own home. This will include working independently in carrying out care plans, rehabilitation, administering medication, providing emotional support and delivering all aspects of personal care. You will have the opportunity to work with a variety of different people on a daily basis providing hands-on assistance, however the emphasis to your role will be to enable people to be independent out in the community.

You will be given training and support to ensure you are able to work in accordance with the required processes, procedures and policies to be a care worker.

Patients

  • To enable patients to maintain their everyday personal skills such as eating, dressing, washing, and hygiene. This may at times include hands-on assistance; however, the emphasis is on independence.

  • Using appropriate moving and handling techniques or assessed/provided equipment as necessary to meet the assessed needs of each individual, at all times respecting their need for independence, dignity and privacy.

  • To support patients with varying health conditions, working in partnership with internal and external agencies, ensuring that individual needs are assessed and met.

  • To enable patients to manage their own medication and be aware of procedures available to assist with this.

  • To be aware of individual goal setting by therapists at MDT and how this contributes towards enabling independence.

  • To use a creative approach in helping people to retain or regain their leisure, social skills and interests in line with the care plan. This includes enabling patients to maintain or regain contacts with relatives, friends and the local community, including being involved in sporting, cultural and religious activities.

  • To offer support and encourage patients to enable decisions based on informed choice and to promote self-advocacy or the use of an advocate, aiming at all times for them to achieve maximum independence.

  • To liaise while on duty with Senior Team regarding any changes to caseloads

  • To observe, record and report, any changes in individual conditions and circumstances and ensure that they are reported to senior staff without delay.

  • To undertake risk assessments before providing assessments to ensure that the individual is able to participate and able to continue with goals set.
